   (2)    Next-fastest:  To order by money order or check, just mail your remittance in the Total Amount of $57.90 (which includes all shipping and handling) per these instructions, to the address below.  Please make your remittance payable to:  Travel.   And then enclose it with a completed copy of the Order Form, below (or the information that it specifies), and mail to the address that it provides.

The following applies ONLY to these residents or citizens:

OHIO and IOWA residents:  Please add 7% state & local sales tax, for a total of $60.88 (which includes p&h).

CANADIANS:  Payment must be by money order or bank draft in U.S. Funds.  Total of $61.90 (which includes p&h).
